Arrangement Preparation & Payment Policy

We will need the following information for the deceased when making the arrangements at the funeral home:

  • Full Name
  • Current Address
  • Date of Birth
  • Place of Birth
  • Date of Death
  • Place of Death
  • Social Security Number
  • Marital Status 
  • Spouse’s Maiden Name (when applicable)
  • Father’s Full Name
  • Mother’s Full Maiden Name
  • Occupation 
  • DD-214 (Military Discharge Papers) (when applicable)
  • Race and/or Ethnicity
  • Highest Level of Education
  • Life Insurance Policies
  • Pre-Need Papers (when applicable)
  • 1 Picture for Memorial Folders, Website and Obituary
  • Information for Obituary (graduation dates, clubs and organizations, church member, interests and hobbies)
  • Who is officiating the service and phone number
  • List of Pallbearers and Honorary Pallbearers with phone numbers
  • Clothing including undergarments; we prefer long sleeves and high neck line shirts, blouses or dresses
  • Survivors list with spouses names and where they currently live
  • Music list that will be played during the service
  • Donations In Lieu of Flowers

Payment  Policy

To lose someone you love is a devastating human experience. It is traumatic and can seriously impact one’s judgment. At Coffman Funeral Home & Crematory, we believe in the importance of the recognition and meaningful celebration of a life that has been lived and we will assist your family in arranging an appropriate service to meet your individual needs. It is also extremely important to be realistic in respect to the funeral expenditures. We do not want a family to spend more than they can not afford nor less than they would like. What is important is that the arrangements be in keeping with a family’s wishes and budget. Provisions for payment are due at time of arrangements. To avoid any possibility of misunderstanding or embarrassment to your family and for your convenience, we accept the following methods of payment:



  • Cash 
  • Verifiable Check 
  • Insurance Assignment of a verifiable policy, must check the status of policy before arrangement with the contact information for us to verify


Payment is due upon signing of the Statement of Funeral Goods and Services Contract. Requested services will continue once payment is secured.

 

Accepted forms of payment:

  • Cash
  • Local Bank Check, return check fee of $35.00 if not valid
  • Cashier’s Check
  • Money Order
  • Valid Insurance Assignment

If you choose to use an insurance policy as payment, we utilize a third party that handles our insurance claims. They charge a percentage of the total amount of funeral/cremation service and the percentage will be deducted from the insurance policy.
